Terms/Agreements: Your credit card information will be processed securely and will not be shared externally.

We require a minimum of 24-hours notice with any scheduling changes. No shows, reschedule or cancellations within 24-hours of appointment will be automatically charged an additional $50 Cancellation Fee to any card on file associated with the patient. Any appointment deposits are non-refundable and will be applied toward your treatment cost. Appointment deposits are 100% transferable to a rescheduled appointment with at least 24-hours notice and should you reschedule your appointment within 30-days from your original appointment date, unless otherwise stated.

Reinvi MD or any of it’s associated companies are authorized to charge any card kept on file for the patient for any of the following; treatments, subscription agreements, commitments to any memberships, package pricing, treatment visits, no-show policies or monthly pay-as-you go payment options. These will be billed automatically without notice on designated billing cycle.

In the event of delinquency you owe the entire outstanding amount on any of the above stated at any point. Further, you will be charged any applicable legal or collection fees in addition to a $50 late fee charge per day that you are delinquent.
